Case Management

Having a Nurse Case Manager is not needed for everyone. However, having a Social Worker such as a Case Manager for an aging loved one can alleviate the stress and time constraints often faced by families coordinating care. These professionals assess the individual's physical, social, and psychological needs, facilitating tailored care plans. Acting as liaisons between the individual, family, and healthcare providers, Case Managers ensure effective communication and appropriate care levels. They coordinate appointments, transportation, and connect individuals with community resources, easing logistical burdens. Moreover, case managers offer emotional support, guiding families through the challenges of caregiving and providing reassurance during difficult times.

The presence of a Case Manager can significantly enhance the quality of care for aging loved ones by streamlining coordination, facilitating access to resources, and offering crucial emotional support. Their role encompasses comprehensive care management, tailored to the individual's needs, ultimately promoting independence and comfort.

Healthcare Advocate

Transportation to and from medical appointments such as doctor’s, OT, PT, Specialty Services, while acting as your health care advocate and liaison between medical providers and family members, with 24/7 medical emergencies response availability, including after-hours checks and advocacy at the ER. Case Managers can also assist with coordination of alternative housing utilizing Senior Placement Specialists.

Wellness Visits/Checks

Providing In-home evaluations of health, social, and safety needs. We work closely with facilities for those living in LTC or loved ones who remain in the home to ensure you or our loved ones are being properly cared for. We help to avoid issues such as isolation, self-neglect, and possibly self-harm through coordination of community services and outings to stimulate social needs. We can provide checks as often as needed.

Crisis Intervention/Family Mediation

Problem solving with families, client advocacy, and crisis management including supervised visits, elder abuse prevention and intervention, coordinating consultation with legal representation or a financial advisor, help accessing local resources, dealing with insurance issues such as claims and applications, assistance with coordinating tax preparation, etc.

Shopping, Errands, Administrative, FUN!

Grocery shopping (online or in-person), clothing purchases, assistance with purchasing needed medical supplies or equipment, prescription pick-up and delivery, transportation to personal service appointments such as beautician, massage, professional and pet appointments, assistance with government benefit applications and renewals, and so much more!
